VIEW COMPLETED NOTE

Patient was admitted to HHS on 2021-07-13, with Dx COPD. Dr. Gregory House referred patient to HHS for SN, PT, HHA services. SN performed skilled observation & assessment, monitored vital signs, assessed mental and physical status, evaluated compliance, effectiveness and side effects of medication, diet and nutritional status, and patient’s and caregiver’s knowledge of signs and symptoms of complications; assessed home for safety to prevent falls, and educated patient on medications uses and side effects, disease process, diet, MD follow up, safety precautions, and emergency management, to dial 911 in case of emergencies such as chest pain, severe SOB/Pain. SN visited patient for assessment, teaching and medication management, twice a week to achieve stated goals in POC.

WOUND CARE SETTING

SN performed skilled observation & assessment, monitored vital signs, assessed mental and physical status, evaluated compliance, effectiveness and side effects of medication, diet and nutritional status, and patient’s and caregiver’s knowledge of signs and symptoms of complications; assessed home for safety to prevent falls, and educated patient on medications uses and side effects, disease process, diet, MD follow up, safety precautions, and emergency management, to dial 911 in case of emergencies such as chest pain, severe SOB/Pain.  Wound care done to right knee as follows : cleanse with N/S, pat dry, apply triple antibiotic, DSD, and secure with tape daily, was done as ordered by MD. Wound has healed, patient is being discharged from Home Health Services with all goals met.

IV THERAPY SETTING

Patient was admitted to HHS on 2021-07-13, with Dx COPD. Dr. Gregory House referred patient to HHS for SN, PT, HHA services. SN performed skilled observation & assessment, monitored vital signs, assessed mental and physical status, evaluated compliance, effectiveness and side effects of medication, diet and nutritional status, and patient’s and caregiver’s knowledge of signs and symptoms of complications; assessed home for safety to prevent falls, and educated patient on medications uses and side effects, disease process, diet, MD follow up, safety precautions, and emergency management, to dial 911 in case of emergencies such as chest pain, severe SOB/Pain. SN prepared and administered  Vancomycin 750 MG/ 250 mL NS over 90 minutes via PICC line and gravity A- Flow to control rate of infusion at 167 mL/hour, Meropenem 1 GM/100 mL NS IV over 60 min via PICC line and gravity as ordered by MD; IV therapy has been completed without difficulties, IV line was removed by Victoria Ray. No further services needed, patient is discharged from Home Health Services with all goals met.

IM/SC THERAPY SETTING

SN prepared and  administered Ceftriaxone 1GM IM daily for 7 days as ordered by MD; IM therapy has been completed without difficulties. No further services needed, patient is discharged from Home Health Services with all goals met.
